We are searching for a new Case Manager to join our team, responsible for facilitatingthe interdisciplinary plan of
care for our patients, with a focus on evaluating the appropriateness of clinical care, medical
necessity, admission status, level of care, and resource management. The Case Manager
RN will coordinate activities that promote quality outcomes and patient
throughput while supporting a balance of optimal care and appropriate resource
utilization and will identify potential barriers to patient
throughput and quality outcomes and will facilitate appropriate discharge
Education: Graduate of an accredited
Experience: Must possess the knowledge and
skills necessary to effectively meet the needs of the patient/internal &
external customer. Two years experience in acute care setting required and Case Management experience preferred.
