OVERVIEW:

Experienced Caregiver/Human Services/Life Coach - PT/$15 per hr. - needed for an independent male in Boulder. Perfect for an active mature individual who achieves goals with little guidance, is attentive, creative, seeks improvement and likes to have fun.
Must love to explore mountain towns and parks, Zoo and other animal sanctuaries, goes to festivals and is a companion on a yearly vacation - all expenses paid.
Participates in planning monthly and weekly activities.
Helps with home care, shopping, medical appointments and foot care.
Staff is responsible for maintaining person's weekly and monthly schedule. This includes scheduling activities of the individuals choosing in advance, budgeting for the months activities, scheduling and transporting to and from appointments, scheduling weekly household tasks such as chores and grocery shopping.
The ideal candidate is outgoing, can think quickly on their feet, has strong time management skills, is organized, detail oriented

POSITION

Various Shifts: Flexible, Full-time or Part-time, Weekends, Morning, Noon, night, etc.

Locations: Various slots available throughout Colorado, can serve in more than one area. Currently needing individuals for clients in Aurora, Denver, Colorado Springs, Littleton, Northglenn, Brighton, Lakewood, Centennial, Thornton, Sterling, Hillrose, and many more.

Training: Training is available to candidates that are interested in making a difference in the lives of our community members.

The Direct Support Professional is a caregiver that provides supports to individuals with intellectual and developmental disabilities in their community and at home. Direct Support Professionals work closely with the individual's team to help the individual reach their fullest potential and personal goals. Assist with feeding, toileting, showers, hygiene, etc. They assist with developing and implementing recreational and activity-based support programs and guide and assist the individual to meet their goals and needs while increasing independence. The Direct Support Professional also communicates frequently and timely with all team members and families and submits weekly documentation.

SUMMARY Primary function is to support developmentally disabled persons living in a personal care alternative environment. Assist with integration/inclusion of clients into the community and methods of instruction, guidance and support in meeting individual needs, with the ultimate goal of increasing independence.

Essential Duties and Responsibilities

  • Assist with meal planning and grocery shopping.
  • Transportation to and from grocery store and some assistance with meal preparation as needed.
  • Works in conjunction with supervisor and other Interdisciplinary Team members to develop and implement habilitative programs (ISSP) for clients.
  • Assist with activities of daily living, such as personal hygiene, laundry and cleaning as needed.
  • Accurately set medications in a medication minder box while following medication minder box rules, including ensuring labels on medication minder box match the current MAR.
  • Maintain current certification in all required courses outlined by Health Care Policy and Finance Division for Intellectual and Developmental Disabilities.
  • Provide accurate documentation of client ISSP as specified in the program, document client needs and supports a minimum of one time weekly in T-logs, write incident reports as necessary, document upcoming medical or psychiatric appointments and obtain signatures of medical professional during appointment.
  • Assist with arranging and transporting to medical or psychiatric appointments.
  • Communicate regularly with supervisor to discuss additional supports and services needed.
  • Submit receipts to supervisor to account for money spent (client grocery money, petty cash, client personal needs funds and other as specified).
  • May perform other duties as assigned

  • Must be able to pass a criminal background check
  • Must have the ability to use a computer and access to a secure network
  • Must have reliable transportation, driver's license, car insurance, and clean MVR
  • Education and/or Experience: High School diploma or equivalent.

    Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Ability to understand and correlate written programs with the appropriate tracking mechanisms for monitoring purposes. Ability to develop computer literacy for tracking/monitoring programmatic goals.

    Reasoning Ability: Ability to apply common sense understanding in carrying out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in routine situations. Spontaneous problem solving ability in certain situations with relation to behavioral considerations of clients both in-house and in various community environments.

    Attitude: Positive attitude and willingness to perform all assigned tasks in a cooperative diligent manner is prerequisite.
