As a Social Work Care Manager you need to know how to:
Coordinate the care and services for patientsidentified as needing assistance or meeting Care Management criteria. Collectin-depth information about a patient s medical, functional, and socialcondition, to identify individual needs in order to develop a plan to meetthose needs. Work with patient and family/caregivers to determine specificgoals and actions based on assessment. Coordinate discharge planning. Executespecific interventions to meet established goals. Organize, integrate andcoordinate the necessary resources to accomplish the goals and plan.
Assess the patient s prior level of functioning,access to and/or use of community resources and available support systems.Assist the care team in developing a plan of care which includes, but is notlimited to: assuring appropriateness of services and care setting, assuringindividualized support and education, determining the need for continuedservices, planning for discharge, and identifying and connectingpatients/families with available community resources if needed. Collaboratewith Physicians and other members of the health care team on the patient sbehalf.
Identify appropriate admission and continuedstay issues. Enhance the quality of patient care through effective and efficientuse of resources. Help to identify strategies for reducing length of stay andappropriate utilization of services. Utilize criteria including clinical pathwaydata and implement strategies to resolve controllable variances.
Attend, facilitate and participate in rounds andcase conferences. Advocate for patient rights. Monitor efficiency and availabilityof services and evaluate outcomes through variance tracking, data collection.
Identify needs, facilitate, or provide educationto physicians and ancillary departments/nursing units regarding casemanagement, discharge planning process, and roles. Participate inmultidisciplinary groups and development of guidelines.
Promote mission, vision, and values of SCLHealth, and abide by service behavior standards.
Perform other duties as assigned.

Graduate of an accredited school of Social Work,Sociology, or related field, required
Minimum of one (1) year of clinical experience,required
At least two (2) years of case managementexperience in an acute care setting, preferred
Master s Degree in Social Work (MSW), preferred
Current Colorado Social Worker license orability to obtain, preferred
