Job Description
SALMON Adult Day Health at Northbridge offers an inspired weekday
program for seniors living at home combining healthcare and social
support with door-to-door transportation available. Clients enjoy hot,
nutritious meals, recreation, personal care and assisted showers, with
other professional services arranged as needed. SALMON s award-winning
Northbridge campus also features Whitney Place Assisted Living and
Memory Care; 5-Star CMS rated Beaumont Rehabilitation and Skilled
Nursing Center; and SALMON Center for Early Education. The variety of
services allows clients and residents to have their changing care
needs met on the same campus. SALMON Center for Early Education offers
staff members first priority and discounts on their child care, along
with convenience and peace of mind. The on-site Center also allows
intergenerational programming to take place, enriching the experience
of the youngsters and residents alike.
Description:
The Adult Day Health Center Program Director is responsible for all
aspects of the operation of the Adult Day Health Center. This
includes, but is not limited to, compliance with all applicable
regulations, financial responsibility, and external relations and
staffing.
Responsibilities:
Conducts job responsibilities in accordance with the standards set
forth in accordance with The Salmon Family of Services policies and
procedures, applicable federal and state laws and applicable
professional standards.
Ensures that employees receive compliance training and education
necessary to perform their job responsibilities, as required by The
Salmon Family of Services policies and procedures.
Works cooperatively as a member of a team.
Keeps confidential all patient information received.
Has authority, responsibility and accountability for the all
functions of the Adult Day Health Center.
Develops and/or maintains policy and procedure manuals, and job
descriptions for all Adult Day Health personnel.
Develops budget for department and recommends to Executive Director
for approval. Decisions made are based on business objectives for
profitability, census, maximizing payer sources and standards of care.
Assures the availability of adequate equipment and supplies.
Establishes systems for care management including assessments, care
planning, and discharge planning. Makes necessary changes to
procedures, programs, etc., to promote practices that are responsive
to members current and long-term needs, department objectives,
standards of practice, and Company policy.
Coordinates member care needs with other departments.
Assures the maintenance of a system for accurate timely and required
documentation, which reflects healthcare needs of every member,
treatment provided, and response to care and disposition.
Delegates supervisory authority and accountability as necessary to
direct reports who ensure fair and objective treatment of Adult Day
Health personnel.
Supervises all department employees disciplinary proceedings.
Maintains required care systems to assure a consistent standard of
care for all members.
Communicates with physicians to assure timely interventions.
Represents the facility to members, physicians, family members of
current or potential members and other community referral agencies.
Encourages customer service orientation in employees.
Strives to enhance team productivity and morale by promoting staff
participation in long-range goal setting by embracing the
organization s vision and mission.
Quickly addresses and resolves performance problems by clearly
communicating expectations, monitoring performance and acting to
resolve issues.
Responsible for marketing and outreach initiatives.
Supervises staff of the Adult Day Health Center including decisions
relative to hiring, training, scheduling, evaluations, attendance,
salary administration, discipline and discharge
Assures orientation is provided and all Adult Day Health personnel
meet competency standards.
Develops individual staff members by helping them to achieve
long-range professional goals.
Assures annual performance reviews and competency testing for
department. Recruits and hires staff.
Champions new ideas for services, programs, approaches and gains
buy-in of staff and up-line managers.
Overseas transportation and safety of transportation including
maintenance, cleanliness, safety compliance. Route development and DOT
compliance for drivers and equipment.
Performs other duties as assigned.
Requirements:
Ability to communicate effectively in English through verbal and
written means.
Knowledge of state and federal regulations and JCAHO standards.
Experience as in an Adult Day Health center of comparable size and
complexity is preferred.
Understands principles of growth and development for the entire
lifespan with the ability to provide care appropriate for patients as
appropriate to the patient population of the Whitney Place Adult Day
Health Center
A valid Massachusetts driver s license.
