Care Manager - Discharge Planner RN

Serves as a patient advocate/liaison, overseeing the continuum of care for the patient from hospital admission to discharge. The RN care manager will take a proactive role by early intervention and collaborative practice with staff nurses, physicians, ancillary services, payer sources, and outside agencies assuring appropriate use of resources while focusing on positive outcomes. Works with the UR team to assure patient is being treated in the most appropriate level of care; documents these reviews in the EMR. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area. Minimum Education/Training:Bachelor's Degree in Nursing or health care related field;consideration may be given to experienced candidates actively pursuing a Bachelor's Degree.Effective January, 2016, any newly hired RN without a BSN will be required to obtain a BSN within 5 years of hire. Exceptions will be made for equivalent experience with Director approval Registration/Certification or License Required:RN - Licensed to practice in the State of Michigan (or completed by the end of orientation)Current BLS for the Healthcare Provider or completed within 60 days of hire. Preferred Education:Masters Degree in Nursing, or related field. Preferred Experience:5 years of Case Management experience in a health care field.Experience with 3rd party reimbursementPhysical Requirements:Occasional climbing, lifting and carrying objects.Occasional push/pull, twisting, bending and reaching.Occasional squat/kneel/crawl.Occasional wrist position deviation and keyboard use.Frequent sitting, standing and walking.Continuous talking and hearing. </script>
