GENERAL SUMMARY: The Physical Therapist, under orders from a physician, evaluates and treats patients who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease, or other causes. As a member of the interdisciplinary care team, the Physical Therapist will assist with prevention and wellness services through screening, consultation, education, and other collaborative efforts within the facility. PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
Evaluates patients and develops appropriate and effective plans of care
Under physician s order, provides patient treatment in accordance with the established plan of care
Assists with prevention and wellness services through screening, consultation, education, and other collaborative efforts with the facility
Attends and contributes to department and facility meeting as requested
Adheres to facility policies and procedures and participates in facility quality improvement and safety programs
Completes documentation effectively and timely, in accordance with facility, Medicare and other reimbursement standards
Demonstrates commitment to providing excellent services in all assigned activities to ensure that the provided services and outcomes meet or exceed the expectations of those utilizing these services or impacted by the department
Adheres to all Laws and Rules as set forth by the appropriate State Practice Act
May be assigned additional duties as determined by the facility Rehab Director KNOWLEDGE AND SKILLS REQUIRED:
Ability to type 40 wpm
Excellent verbal and written communication and interpersonal skills
Attention to detail, ask questions about things that do not appear to be correct
Strong time management and problem solving skills
Highly organized
Competency in the field of Physical Therapy including independent and effective evaluation and treatment skills, operation, application, and care/maintenance of equipment, and the ability to assess patient needs and develop and implement a comprehensive plan of care
Proficient in the use of a computer
Ability to read, write, and speak English EDUCATION AND EXPERIENCE REQUIRED:
Bachelor of Science Degree in Physical Therapy
Current Physical Therapy license in the state hired SUPERVISORY RESPONSIBILITIES
