General Description: Draws blood from assigned patients at daily assigned long-term care facilities. Delivers assignments to approved designated drop off location. Follows all protocols and practices and appropriate caution when handling specimens in compliance with ACULABS Blood Bourne Pathogens Exposure Control Plan . Treats any specimen as though it may contain blood borne pathogens.

Previous phlebotomist experience preferred.

Job Role

  • Verifies the legitimacy of requests for patient testing;
  • Performs phlebotomy in response to legitimate requests by a physician or other medical professional received directly or through established ordering procedures (repetitive orders, daily or one-time orders or STAT requests);
  • Follows appropriate procedures for identifying patients on whom phlebotomy is to be performed; confirms patient ID by utilizing 2 active identifiers;
  • Draws blood specimens and collects specimens at daily assigned long term facilities. Performs STATS, Timed Tests, Fasting Tests, Redraws, and UTO s as assigned by Dispatcher, QA Supervisor and/or Regional Supervisor;
  • Makes appropriate entries on the test requisition attesting to the success/failure of phlebotomy, receipt/inability to locate specimens other than blood. Phlebotomist initials and enters their unique ID number, date of service and time of draw. The signature of the ordering physician/nurse must be noted on the requisition;
  • Informs unit nursing personnel and obtains signature documentation in the following situations: patient is uncooperative; phlebotomy is unsuccessful for any reason; requested specimens cannot be located; test requisition is incomplete (missing required information or diagnosis); or, patient s ID is in question;
  • Responds to requests for scheduled or STAT phlebotomy (at any long-term care facility) in a timely manner and at a Dispatcher s and/or Supervisor s request;
  • Adheres to assigned schedule and completes work in an efficient and timely manner;
  • Utilizes all prescribed skills and techniques in obtaining a viable blood specimen for testing without causing pain or undue discomfort to patient. After 2 venipuncture attempts, advises nurse that attempts were unsuccessful;
  • Exercises all prescribed precaution in handling blood or fluid which may expose him/her to blood borne pathogens;
  • Adheres to commonly accepted Infection Control protocols (i.e. gloves and washing hands before and after patient contact).;
  • Delivers all collected samples to laboratory or designated collection site in a timely manner;
  • Ensures integrity of specimens is maintained during transport to laboratory or collection site;
  • Uses only those Biohazard protection devices and containers for sharps that are approved by ACULABS and meet OSHA exposure prevention standards;
  • Interacts with nursing personnel at long-term care facilities in accordance with ACULABS policies and ensures courteous and professional behavior;
  • Arrives at work at the prescribed time (i.e. start times vary between 5:00am and 5:30 am depending on the facility. Please check your scheduled start time on your new hire sheet.);
  • Dresses appropriately for phlebotomy at a location that is a medical facility or the patient s residence Scrubs , and lab coat, or uniform;
  • Maintains his/her equipment, restocking in appropriate manner;
  • Ensures biohazard materials are disposed of according to phlebotomy policies and procedures;
  • Required to fax all test requisition forms, UTO/Refusal forms, Redraw forms, etc. to the laboratory prior to leaving each facility (Consult with your direct supervisor for full instructions on faxing forms);
  • Submits Daily Mileage Sheets and/or utilizes remote time management program as directed;
  • Drives to long-term care facilities, laboratory, and/or collection sites using personal vehicle;
  • Follows all policies and procedures outlined in the Phlebotomy Manual, Employee Manual and Safety Manual;
  • Requirements

  • High School or equivalency diploma;
  • Valid driver s license and valid insurance/registration during the duration of employment;
  • Must provide own transportation and be able to travel from location to location in accordance with daily schedule;
  • Successful completion of phlebotomy exam (80% and above);
  • Phlebotomist will be issued a company phone which must remain on throughout their entire work shift;
  • Skills and competency will be tested prior to start date.
