AREA OF ASSIGNMENT: Opioid Treatment Network Care Team

HOURS OF WORK: Typically Monday Friday; Hours may vary to meet department needs.

RESPONSIBLE TO: Director, Care Management

PREREQUISITES:

  • Current Washington State Registered Nurse license.

  • BSN preferred

  • Valid Cardio-Pulmonary Resuscitation Card (CPR) preferred.

  • Two years of recent clinical experience as an RN working in a clinic/outpatient setting preferred.

  • Demonstrated knowledge and understanding of information technology.

  • Completion of the American Association of Ambulatory Care Nurses (AAACN) Care Coordination and Transition Management (CCTM) Course within three months of hire.

  • CCTM certification is to be completed within one year of hire.

  • Experience within Behavioral Health desired

  • Experience with Substance Use Disorders preferred

  • Essential Responsibilities and Competencies:

  • Population Management

  • Identifies patient populations requiring care management support for Opioid Use Disorders

  • Assesses patient populations to identify those resources or other factors needed to achieve the desired outcome for health maintenance or health improvement.

  • Coordinates healthcare interventions for populations with significant health conditions in which self-management efforts are critical.

  • Maintains appropriate patient educational materials for populations of patients to meet the needs of patients and families to assist with the facilitation of their participation in the plan of care.

  • Oversees the management and tracking of notifiable illnesses. Tracks communicable diseases when alerted to do so by the health department, VMC infection prevention department or employee health and per clinic directive.

  • Analyzes data related to patient populations/conditions and develops a plan of action. Monitors progress over time and initiates changes as needed.

  • Develops strategies to meet the preventive care and health maintenance measures for populations of patients.

  • Utilizes the Epic Care Team functionality to become part of the care team for identified patient populations.

  • Develops professional relationships with community resources that are used by VMC to care for populations of patients. (e.g. Home health, hospice)

  • 2. Disease Management-Opiod Use Disorders

  • Assists in the management of patients with Opioid Use Disorders following established protocols and systems in collaboration with providers.

  • Conduct screening of clients interested in medications for Opioid Use Disorders

  • Provides client education about medications for opioid use disorders

  • Provides education about safe buprenorphine home induction

  • Administers medications as ordered by provider, including intramuscular and subcutaneious injections

  • Obtains vital signs, urine drug screens and basic labs

  • Conducts physical assessments and provides wound care support

  • Documents in Epic and treatment registry

  • Participate in stabilization and maintanence phases of medication assisted treatment of clients

  • Ensure appropriate care coordination and follow up

  • Support access to higher levels of care if relapse occurs and work with care navigator to ensure relapse prevention support is available

  • Utilize harm-reduction strategies to support patient care

  • Assesses patient learning needs and has the ability to develop and implement individualized educational or care plans. Reviews, evaluates and revises the plan on an ongoing and timely basis.

  • Communicates with a multidisciplinary team (physicians, nurses, therapists, social workers, etc.) as needed to support OUD management

  • Develops self-management goals, care plans and monitors the progress of the goals.

  • Has the ability to oversee and assist the patient with referral navigation.

  • Utilizes patient communication strategies, e.g. motivational interviewing, to involve the patient in developing a plan of care, goals or other specific measures pertinent to their health condition.

  • Assesses patient activation and readiness for change and uses these to develop self-management goals.

  • Documents all disease management encounters using standardized processes.

  • 3. Utilization Management

  • Possesses analytical skills to assess various patient utilization measures, such as ED, Urgent Care and Hospital Visits.

  • Oversees ED, Urgent Care and Hospital admission utilization rates.

  • Collaborates with the Leadership team to develop a plan of action to maintain acceptable utilization rates.

  • 4. Leadership

  • Works collaboratively with the MA or LPN Clinic Coordinator, to promote activities that support the overall goals of the clinic related to caring for different populations of patients.

  • Engages and leads the back office team and partners with leadership to support the population, disease and utilization management process goals and initiatives.

  • Effectively communicates with staff members and providers. Can role model excellent communication skills.

  • The RN Care Manager may provide clinical expertise for back office staff on pertinent patient care issues.

  • Participates in the development and implementation of policies and procedures. Works collaboratively with the leadership team to ensure that the staff comprehend and are compliant with the policies and procedures that relate to population, disease and utilization management.

  • Actively participates in all leadership team activities.

  • Participates in the interview process for back office positions and makes recommendations to the Clinic Manager.

  • Participates in maintaining the fiscal requirements for the clinic.

  • Assists in the performance appraisals of personnel through documentation and input to the Clinic Manager.

  • Facilitates the resolution of patient complaints in a professional manner.

  • Acts as a liaison between clinic, other hospital departments and community resources.

  • 5. Quality

  • Monitors monthly quality measures, looks for trends and makes plans for improvements. Identifies problem areas for monitoring and evaluation and is active in analyzing findings, changing practice based on the findings. Works with the Medical Director and Clinic Manager on process and informs staff of trends and areas where improvement is needed.

  • Serves as an educational resource and provides consultation to other staff on utilizing evidence based critieria to maintain quality measures.

  • Participates, as a Clinic Team Member, in Quality Improvement Projects and Initiatives.

  • Assists with regulatory readiness and staff compliance (The Joint Commission, DOH, HCA, OSHA, etc).

  • Assists with the quality management of clinical services according to clinic network policies.

  • Education, Training and Competency

  • Stays abreast of current and updated clinical practices, policies and procedures. Is able to provide clinical expertise in the development of such documents.

  • Participates in the training of new staff for population management, disease management and utilization management.

  • Maintains job-specific certification.

  • Support training and consultation to RN staff providing MAT/OUD support in the Emergency Department

  • Patient Safety

  • Maintains awareness of changes in the National Patient Safety Goals (NPSG) and incorporates all NPSG standards into practice.

  • Triages according to protocol for emergency situations.
