In this role the NP will receive the patient when admitted to the Nursing Facility and ensure all the records from the hospital discharge are received and ensure the patient medication list is reconciled. If necessary discussions with patient?s family is provided and follow up with test results. All NPs in the practice collaborate with our physicians for the care of the patients in the facility but they see the patient and bill for services independently.

In addition, the NP will provide care to patients with acute and chronic conditions in a Skilled Nursing Facility. The NP will assess patients, order and interpret diagnostic tests, make diagnoses, initiate and manage treatment plans including prescribing medications. Other areas of patient care could include, outpatient practice setting and home care. Other responsibilities include staff and community consultation, health education and health promotion, presenting of undergraduate and graduate students, and facilitating client participation in research.
