Responsibilities:

* Provide orientation and ongoing case management services for court involved and/or out-of-school youth participating in the E-3 Center. Manage a caseload of 20-25 youth who are or returning from residential placement
* Meet with court-involved youth one month prior to discharge from residential placement and obtain information from residential placement staff prior to youth's arrival
* Provide regular updates on court-involved youth's progress to Reintegration Workers and Probation Officers and coordinate services (initially through weekly contacts)
* Meet with youth returning from placement weekly to provide support and monitor activities, or meet with out-of- school you bi-weekly for the first month and then monthly to do the same
* Maintain regular contact with youth's families to help support their participation in the Center, to help reinforce the skills learned, and to identify potential barriers to success
* Coordinate completion of multi-faceted, comprehensive assessment process
* Make referrals to educational, vocational and employment resources in collaboration with the E-3 Center team
* Identify and help resolve barriers to participation in Center activities
* Help youth and his/her family establish linkage to appropriate community resources, including health and behavioral health resources as well as social activities
* Participate in weekly team meetings, staff meetings, and supervision meetings
* Complete appropriate documentation for all services provided
* Maintain E-3 Center database and report outcomes on progress in educational, life skill, vocational training, and work activities on a monthly basis

Qualifications:

* BA and at least two years of directly relevant experience of MA in social work, family therapy, criminal justice of related field
* Excellent writing, communication and organizational skills
* Working knowledge of high-risk youth, behavioral health issues, and family systems
* Strong interpersonal and communications skills
* Ability to work independently and as part of a team
* Sensitivity to racial, cultural, and economic differences.
