Responsibilities:

* Perform client needs assessment, based on intake materials and client interview.
* Provide case management/care coordination and advocacy by carrying a caseload and assisting clients in negotiating social services, providing follow-up and monitoring support. Reassess client need as appropriate.
* Maintain contact with staff at training programs with Interim Houses West client participants.
* Meet weekly with clients enrolled in vocational training programs, to assess their level of satisfaction with the program.
* Meet with client and staff at training program when it is viable to begin to search for employment.
* Assist residents with job applications and applications to vocational and educational training programs as needed.
* Meet weekly with all clients on caseload to address case management issues and concerns including budgeting, entitlement, housing and employment.
* Attend court hearings for clients related to criminal justice and family related issues.
* Consult with the treatment team in formulating discharge planning.
* Finalize plans for referrals for therapeutic support.
* Complete continuing care plan with each resident, 2 weeks prior to discharge from treatment.
* Attend daily staff meetings.
* Attend weekly individual supervisory meeting with the Senior Therapist.
* Maintain accurate, timely records of time spent with client, problem areas, problem resolution, etc.
* Participate in staff development as required.
* Provide client transport to social services and medical appointments according to program phases.
* Coordinate and monitor client use of transpasses and tokens.
* Co-facilitate specialized client groups including conflict resolution, life skills and other duties as assigned.
* Identify housing options for clients with in the first 2 months of treatment and continue follow-up throughout the clients' treatment stay.
* Develop and monitor monthly budgets with clients and identify areas, which will affect clients' ability to access housing post-treatment. (I.e. outstanding student loan payments, credit card debt, utility bills, etc.)
* Complete employment needs assessments with clients on a quarterly basis.
* Other duties as assigned.

Education/Qualifications:

* Bachelor's Degree and 3 years experience in social/human services or Master's degree and one year of related experience.
* Expertise in case management and client advocacy.
* Ability to work with addicted women with critical and emergent situations. Sensitivity to cultural and economic differences.
* Knowledge of regional and service pool.
* Thorough understanding of medical and psychosocial issues related to addiction.
* Valid Pennsylvania Driver's License.
