We are seeking a full-time Case Management Supervisor to provide supervision to (4) emergency housing case management staff that are located within (3) emergency housing programs. The Case Management Supervisor will be responsible for the provision of case management services based on the following principles: Housing First, Housing Focused, Prioritization, Progressive-Engagement, Person-Centered, and Strength-Based. The Case Management Supervisor will ensure that all case management staff adheres to all outlined responsibilities included in the Office of Homeless Services Case Management Standards along with practices/services aligned with the National Association of Social Work's Code of Ethics.

Responsibilities:

* Provide all supervisory duties for case management staff at the designated Emergency Housing program, including scheduling and monitoring of work duties, clinical and administrative supervision, and performance management;

* Provide direct case management support to transitional housing program (Potter's House Mission) and (3) emergency housing programs;

* Carry a caseload of no more than 15 clients;

* Ensures that all housing referrals for program participants are submitted within the outlined timeframe;

* Updates, monitors, and maintains weekly and quarterly reports (i.e., OHS weekly discharge spreadsheet and HAP report) to be provided to the Director of Rapid Re-housing and Case Management Services;

* Monitors adherence to the OHS provision of case management services;

* Identify compliance and production problems, and consult with Director of Rapid Re-housing and Case Management Services for corrective action plans;

* Act as an arbitrator when problems occur between participants and Case Management staff;

* Complete all Healthy Baby Program surveys for all babies between the ages of 0 and 4 months;

* Monitor all early intervention referrals made by provider agency, and maintain collaborative contact between program participants and early intervention staff.

* Participate in interagency meetings (i.e., DHS, FaSSt etc.)

* Act as a liaison for existing and/or new OHS initiatives including the Healthy Baby Program and Philadelphia Infant Toddler Early Intervention Program to ensure client and program compliance;

* Interpret and explain rules and regulations to staff, and provide assistance (where necessary) with managing case management workloads;

* Develop interventions to assist households with obtaining stable, permanent housing;

* Respond to both informal and formal grievances from program participants, outside agencies, emergency housing staff, and OHS staff and relays all pertinent information to the Director of Rapid Re-housing and Case Management Services

* Review and assess all daily client related incident reports;

* Conduct weekly Participant Progress Review Meetings and ensure that all outcomes are documented within the HMIS system;

* Participates in monthly supervision with the Director of Rapid Re-housing and Case Management Services;

* Conducts monthly client record audits of case managers to ensure compliance with OHS Case Management Standards;

* Collaborates with community based and/or social service agencies, as appropriate;

* Provide monthly documented supervision to Case Management staff;

* Adheres to OHS CM standards with regards to completing Health and Safety Discharges and Appeals if applicable for non-compliant participants;

* Assist Case Managers with developing appropriate interventions to assist participants with compliance with Emergency Housing and OHS Case Management Standards and barriers that prevent re-entry to housing.

* Using Sanctuary Model, the Case Manager Supervisor will facilitate monthly community meetings alongside the Shelter Director;

* Completes other duties as assigned

Skills:

* Knowledge of NASW Social Work Standards and both Trauma Informed Care and Progressive Engagement

* Strong management skills

* Ability to documents using SOAP or DAP format

* Ability to effectively use Excel, Outlook, Microsoft word

* Ability to exercise effective advocacy and counseling skills

* Experience with the implementation of the Housing First Model

* Ability to communicate effectively using English in both written and oral forms

* Ability to interact professionally with the public

* Ability to multi-task and respond effectively to policy and procedural changes within the Emergency Housing system

* Prepare comprehensive reports (HAP)

* Manage detailed information efficiently and analyze facts and exercise sound judgment

* Ability to perform basic Math

* Understand English and follow oral and written instructions

* Use current software and computer applications

PHMC COMPLIANCE RESPONSIBILITIES:

* Understands and adheres to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistle Blowers and Conflict of Interest Policies

* Keeps abreast of all pertinent federal, state and PHMC regulations, laws, and policies as they presently exist and as they change or are modified

* Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job

* Comply with Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards

* Adhere to all PHMC Policies and Procedures

* Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module

Experience:

* Prior direct service experience with people experiencing homelessness and/or those with substance abuse or behavioral/mental health issues; familiarity with social, health and benefit resources

* Three years or more of Case Management or Social Work supervisory experiences

Education Requirement:

* Master's degree in Social Work from an accredited college or university
