Job Summary:

Implements, with minimal supervision, high quality occupational therapy services for patients, caregivers and facilities in accordance with the principles and practices of occupational therapy and with Premier policies and procedures. Evaluates patients, develops initial and ongoing treatment plans, develops discharge plans, assists assistants and aides, contributes to case management, and provides quality patient care. Maintains positive level of interaction with facilities and clients. Maintains good client relations with facilities and their staff. Adheres to applicable state regulations concerning occupational therapy.

Essential Functions*:

  • Optimize functional abilities and skills of patients in facilities by performing the following activities: screening of all residents; evaluating and treating all patients who could benefit from intervention as referred by a state approved referral source; complying with Minimum Data Set (MDS) procedures and effective care planning; appropriately documenting progress toward indentified objectives; and communicating patient status and needs to the patient, the patient's family, the staff, and other professionals as appropriate.

  • Refers patients for treatment and consults with Occupational Therapy Assistant's (OTA's) as appropriate by completing evaluation and initial plan of treatment, and communicating necessary and sufficient information.

  • May provide assistance and consultation to Occupational Therapy Assistants and Rehab Aides in skilled tasks during group and individual treatment in a manner that promotes appropriate utilization of resources and facilitate continued developments of clinical skills in accordance with State Practice Acts, Omnibus Reconciliation Act (OBRA), and Premier policy.

  • Adheres to all applicable federal, state, local, and company maintained standards of care and ethics and policies concerned with the practice of Occupational Therapy and personnel qualifications.

  • Is responsible for participating in the following activities: establishing and maintaining courteous and helpful interaction patterns with staff, patients, and families which reflect sensitivity to individual needs and a problem solving approach; actively integrating therapy's role in facility and interdisciplinary treatment team; contributing to facility patient care, utilization review, case management, administrative staff/department head, and family conference activities/communications; establishing and maintaining a good working relationship with other health care professionals and colleagues in the community; making appropriate referrals to other members of the rehabilitation team, developing and implementing new programs/techniques to enhance rehabilitation services; adhering to established facility policies and procedures related to environmental safety, orderliness, risk management, infection control and emergency situations; and adhering to the requirements of the patient's Bill of Rights.

  • Continue professional development by: expanding and enhancing clinical expertise, professional and management skills through interaction with managers, therapists and other professional staff, self-study, and other continuing education activities; maintaining valid state license (where appropriate); obtaining membership and participating in professional and community organizations; maintaining awareness of issues related to the profession of occupational therapy and the health care environment.

  • Maintain administration requirements by: submitting billing, payroll and expense documentation in an accurate and timely manner; assisting in collection of statistics and updating records; informing appropriate staff and managers of patient progress and facility issues; and contributing to facility rehab goals and plans.

  • Understand managed care/Prospective Payment System (PPS) billing and treatment guidelines. Provide quality care as well as reasonable goals and outcomes within the guidelines.

  • Meets productivity and utilization expectations as established by the Company and facility

  • Compliant with infection control procedures and environmental safety protocol within a facility. Must embrace the team concept and interdisciplinary care.

  • Maintain confidentiality of Premier, facility and patient information

  • Some travel between facilities may be required on an as needed basis.

  • Ability to work varying hours, including weekends and holidays.

Required Skills & Abilities:

  • Communication skills sufficient to effectively cross-refer patient treatment to other disciplines after the completion of an initial evaluation and initial treatment plan.

  • Ability to work effectively as a team member.

  • Organizational, time management, and planning skills.

  • Communication skills for accurate, concise, and organized oral and written presentations of information to patients and staff.

  • Listening, reading and comprehension skills for analyzing reports, charts and correspondence.

  • Effective clinical problem solving skills (e.g. identify and prioritize issues in responsive manner, develop sound and innovative action plans, execute action plans on timely basis, ensure appropriate results, etc.).

Required Credentials:

  • Graduate of an accredited school of occupational therapy.

  • Valid state license (where appropriate).

  • Valid driver's license.

Physical Requirements:

  • Ability to lift patients using appropriate biomechanical techniques frequently throughout the course of a work day/shift; must be capable of easily lifting fifty (50) pounds of dead weight alone.

  • Ability to walk, bend, stand and reach constantly during the work day/shift.

  • Visual, auditory and speaking skills sufficient to evaluate, diagnose, communicate, and monitor patient needs, and to maintain accurate records, recognize people and provide and understand written, verbal and gestural communications.

  • Ability to push wheeled equipment throughout the facility and in the community.

  • Fine motor skills for legible and accurate writing of reports, charting, scheduling, daily correspondence, and presentations.

  • Fine motor skills for effective and efficient handling of diagnostic or therapeutic equipment.

  • Ability to participate in sustained activity for many hours in duration in accordance with state labor laws

Clinical Service Delivery and Documentation Expectation:

Premier Therapy expects that all employees will conduct clinical services within the expectations of all Medicare/Medicaid and Third party regulations. In order to maintain state licensure, therapist must stay in compliance with their respective practice acts.

Licensed therapists are responsible for determining medical necessity of therapy services from evaluation established through:

  1. Presence of physician's orders for evaluation and treatment
  2. The establishment of a decline in condition via an appropriately documented objective prior level of function and objective measures of current functioning
  3. The documentation of a complex condition that requires the skills of a therapist,
  4. Establishing a POC that contains the appropriate modalities, frequency/duration, and short/long term goals in measurable format with timeframes.
  5. Conducting ongoing assessments of the patient's progress and continued need for skilled intervention through progress reports(10th visits), updated plans of care, direct patient treatment and, supervision of the execution/documentation of the patient's treatment plan by the therapist's assistant.
  6. Completion of the Discharge assessment which underscore the value of progress made, skilled services provided and ultimate functional impact on the patient's life.

Licensed therapists are responsible for ensuring that the POC they have established meets the technical requirements to ensure appropriate billing of therapist services through:

  1. Timely completion of cosigns (within 7 business days of last treatment completed on the document)
  2. Physician's signature/date on the plan of care
  3. Completion of daily notes, progress reports(10th visits), and UPOCs on the date of service.
  4. Assignment and documentation of Functional Limitation - G codes
  5. All assistants are to assist the above with completion of the aforementioned tasks within the scope of their state practice acts and CMS guidelines
  6. Therapists are expected to document on the date of service and to oversee that assistant documentation is completed daily as well.

Therapists are expected to know and follow state practice acts in regards to continuing education expectations, as well as supervision and co-sign requirements.
