JOB DESCRIPTION
Under the direction of the Discharge Medication Program manager, Discharge Medication Coordinator is responsible to present the Discharge Medication program to skilled nursing facilities that are currently serviced in all the states, and to meet with facility[s key members to introduce the program, give a detailed overview of the benefits of the program to the facility and patients, perform hands on training at the facility, obtain feedback and answer any questions facilities may have with a goal of facility enrollment in the discharge medication program. The Discharge Meds Coordinator will work to develop an understanding of the Discharge Medication program, it s workflow, benefits, and develop a relation with the nursing home customers to ensure ultimate success of the program and a safe discharge for the patient.

Essential Functions:
Present and educate administrators, DONs and LTC facility staff on the new discharge medication program
Coordinate program start dates at facilities by ordering the necessary facility equipment, setting up the facility with needed forms, setting up the correct communication channel by identifying key facility members to ensure success of the program and coordinate an easy program launch
Effectively understand the details of the Discharge medication program workflow and responsibilities of facility, pharmacy and patient roles
Quickly understand benefits of the program and communicate clearly to the facility staff the importance of the Discharge medication program and its direct connection to lowering facility s readmission rates
Partner with assigned nursing facilities to ensure customer satisfaction with Discharge Medication program and to receive feedback
Effectively collaborate with the Discharge Medication team in the pharmacy throughout operations to deliver additional value to facilities
Promote Discharge Medication program and advise facility staff on billing coverages, returns and other related issues
in-service, inspect, advise and troubleshoot any program related needs necessary on site in the facility
Provide a variety of approved in-services on site and telephonically
Entry-level experience with Microsoft Office programs such as Word, Excel, and Outlook

Work Environment:
This position operates in a long-term care facility. This role may use standard office and/or pharmacy equipment such as computers, phones, photocopiers, filing cabinets, rolling carts, fax machines and etc. A large portion of work shall be performed in the long-term care facility where the noise may be loud.

May sit seven (7) to ten (10) hours per day
May be necessary to work extended hours as needed
