General Responsibilities: Promotes Cranes Mill to the community, local hospitals, medical centers, physicians, case managers, social workers and other healthcare professionals to maintain a healthy census throughout the building with emphasis on the health center. (SNF, ALF, MSU) Serve as advocate for patients beginning and during the preadmission process and continuing until the patient is discharged from the medical facility. Assists the Marketing Department in promoting the entire Cranes Mill community to the local area through participation on local boards, community groups, neighborhood block associations, and other organizations. Represents Cranes Mill in local organizations. Serves as patient advocate when residents are hospitalized. Essential Duties: Supports the mission and values of Lutheran Social Ministries of New Jersey, which is: Through the power of the Holy Spirit, and in response to Gods love as revealed in the Gospel, the mission of the Lutheran Social Ministries of New Jersey is to serve those in New Jersey who hurt, are in need, or have limited choices. Drive revenue and meet/exceed associated sales goals. Develops relationships in the community on behalf of Cranes Mill. Establishes patients eligibility for care and communicates with families of patients while consistently communicating with health center team at Cranes Mill. Act as a liaison between Cranes Mill and healthcare facility when preparing for discharge. Ability to complete admission process as needed and upon request Coordinate time between the hospitals, rehab centers, etc. External daily outreach should be no less than 6 hours per day. Submit weekly update and structured schedule to supervisor with strategic plan for the upcoming week. Perform tasks related to logging daily sales activity in CRM database. Communicate internally about new opportunities, service issues and other issues that affect the day to day business of a clinical liaison. Maintain and develop existing and new customers through appropriate propositions and ethical sales methods and relevant internal liaison to optimize quality of service, business growth and resident satisfaction. Plan and prioritize personal sales activities and customer/prospect contact towards achieving agreed business goals, including costs and sales- especially managing personal time and productivity. Manage and control expenditures within established budgets. Actively participate in community organizations to promote the presence of Cranes Mill and to assist in the development of a viable, active referral base. Prepare and execute external presentations individually or with the marketing team Actively participate in all Special Events on and off the Cranes Mill Campus Coordinate Special Events & Open Houses for professionals to enhance awareness of Cranes Mill and its Health Center in the marketplace. Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties. Adheres to the Organizations Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns Follows all local, state and federal regulations as they pertain to the position. Adheres to to the Organizations Policies and Procedures covering Privacy and Security in Complicance with HIPAA regulations. Other duties as assigned by my supervisor, necessary for the efficient operation of the department/facility. Education/Training/Certifications: Preferred: Bachelors Degree in a related field; Social Worker, RN or LPN from an accredited school. Valid NJ Drivers License required. Experience: Sales experience in a healthcare setting. Skill(s): Relationship building and negotiating skills required. Excellent organizational & time management skills Detail oriented & able to handle multiple tasks simultaneously A self-starter and able to think & exercise independent judgment. Able to respond to customer requests in a timely manner An enthusiastic person with sincere compassion for the senior population. Professional appearance & proper phone etiquette without fail Computer Expertise Required: Microsoft Office, PowerPoint, and Marketing Database software Dependable & have flexibility with work hours Strong communication skills (oral and written) and excellent interpersonal skills. Maintain a positive and professional working relationship with all individuals internal and external to the Community. Local Travel required at minimum 60% of the time.
