Center For Advanced Heart Failure & Pulmonary Vascular Disease

The Heart & Vascular Institute's Center for Advanced Heart Failure & Pulmonary Vascular Disease has developed extraordinary resources to help patients with advanced heart failure, whether it's with life-support ECMO technology during cardiac or respiratory failure, heart pumps that restore normal blood flow or a heart transplant.

Advanced Practitioner Role Scope

The Advanced Practitioner will work at Hartford Hospital within the Center for Advanced Heart Failure providing comprehensive ambulatory care in our heart failure management program. This office based Advanced Practice role is ideal for an individual comfortable working within a fast paced clinical setting as part of a multidisciplinary group of providers including APRNs, physicians, nurses and dieticians. The outpatient heart failure management program incorporates a HF clinic, disease management program, CardioMEMs program and HF infusion center to actively manage heart failure related volume overload and reduce hospital admission. The provider should exhibit a passion for cardiology and be open to a rapidly growing practice where adaptability is paramount. Schedule: Monday through Friday; flexible days and hours

  • Qualifications

    Advanced Practitioner Qualifications

  • Graduate of an accredited Physician Assistant / Nurse Practitioner Master s program preferred
  • Licensed, or eligible to be licensed, as a Physician Assistant / Advanced Practice Registered Nurse (APRN) in the state of Connecticut
  • Certification or eligible to be certified as a Physician Assistant or National Board Certification as an Advanced Practice Registered Nurse
  • Cardiology experience preferred
  • Experience with heart failure or VAD program preferred
