Einstein campuses are Tobacco and Smoke Free.Einstein is currently offering a $10,000 sign on bonus for Registered Nurses in our Care Management department with at least one year of recent acute Care Management experience who are hired into a full time position.In this role you will assume immediate responsibility for:Deciding on necessity of medical attention, admission suitability, continuing stay and what type of care is needed based on clinical information and third party informationDetermining admission and continuing stay criteria with third party payers and Physicians. Recommending other options when necessaryAssisting on the effective prevention and management of denials in regards to the appeal processPerforming necessary interventions for discharges at home with self-care, home with services or short term skilled nursing facility placement. Assembling referrals, discharge summaries and applicable information for discharge to homeEnthusiastically giving to, contributing to, and following through on interventions identified in care coordination and patient roundsImplementing and assessing an effective discharge plan in collaboration with the multidisciplinary teamsSupporting patients in determining a discharge planAddressing barriers for discharge If you possess the following qualifications, please apply immediately:Bachelor's degree in Nursing requiredCurrent Pennsylvania RN license, active and in good standing is requiredCPR certification through the American Heart Association requiredOne to two years of discharge planning experience requiredOne to three years of clinical experience requiredLong term care experience preferredOne to two years care management experience preferredGeneral knowledge of utilization management within a hospital or managed care organization is requiredKnowledge of managed care contracting preferredExcellent communication and customer service skills requiredWorking knowledge of PC applications required
