Einstein campuses are Tobacco and Smoke Free.In this role you will be responsible for:Provides services to assist patients and their families meet their biopsychosocial needs by enabling them to access and utilize medical care and community resourcesThe goal is to achieve expected functional outcomes with specific regard to the patient's age and physical limitationsProvides medical management coordination to enhance the efficiency of delivery of care by improved and prioritized scheduling of services and appropriate allocation of resourcesProvides consultation and teaching to the team as neededThis position requires providing service to the following age population(s): AD, A, O, F in a manner that demonstrates an understanding of the functional/developmental age of the individual servedIf you possess the following qualifications, please apply immediately:MSW or MSS requiredBSW may be considered based upon experience and knowledge of care managementPosition requires three to five years of clinical experience in a hospital, preferably in a rehabilitation hospitalLSW optionalPhysical Demands:Working Conditions: Generally environmentally controlled climate. Occasional trips to the community for home visits or meetings are required. Position requires extensive telephone usage and occasional computer usage.Physical Requirements: Light. The position requires standing, walking and sitting. Additional capabilities include bending, crouching, fingering, gripping, kneeling, pulling, pushing, reaching, stair climbing. Excellent listening and verbal skills required. Average visual acuity required.
