Works collaboratively with the Medical Director/Executive Director of the CFDT and Outreach team to foster obstetric referral relationships with physicians, genetic counselors, nurse midwives, and payers locally, regionally, nationally and internationally for the CFDT. Responsibilities include establishing systems/processes to ensure excellent customer service to referrers; facilitating communication between the CFDT and referrers/institutions and facilitating continuing education programs. The Nurse Coordinator for Outreach is responsible for representing the Center for Fetal Diagnosis and Treatment (CFDT) and the Fetal Heart Program (FHP) to maternal fetal medicine and obstetric physicians through relationship building and educational marketing to increase utilization of CFDT/FHP services. The Nurse Coordinator for CFDT Outreach has subject matter expertise in areas of referral needs and provides support to the CFDT and FHP in enhancing these relationships. This position supports the strategic objectives of the CFDT/FHP and CHOP.
Ensures effective communication regarding patient progress of patients referred to the CFDT to referring physicians and hospitals. (15%)
* Informs referring physicians of patient progress and expected care plan while in-house for newly admitted NICU patients as appropriate.
* Facilitates attending-to-attending physician communications as appropriate.
* Provides follow-up on all maternal and neonatal patients to ensure referring physician satisfaction.
Maintains contacts with referring physicians to ensure excellent customer service for referring physicians and hospitals. (50%)
* Assists affiliated hospitals with obtaining resources and information from CHOP as requested.
* Advises existing referring sources of new programs, service developments, as well as changes in the referral process.
* Facilitates sub-specialty consultation with other clinical services (e.g., cardiology, neurology and CT surgery).
* In collaboration with Director and NIC Medical Director, addresses concerns and issues raised by neonatologists regarding CHOP policies and plans.
* Schedules and executes a minimum of 8 NICU/OB/MFM visits per month and 25 follow-up phone calls per month.
* Site visits should consist of 80% high/average volume and 20% low volume referral sources.
Establishes interdisciplinary outreach team approach. (10%)
* Collaborates with CFDT Outreach committee and Executive Director to craft annual Outreach plan to meet established growth targets.
* Develops process to communicate and coordinate neonatal outreach initiatives among the NIC, CICU, Transport, and CFDT staff.
Collaborates with CFDT Executive Director, NIC and Outreach Medical Directors, incorporating feedback from CICU and NICU, to develop a business plan to identify and capture new referral sources from the CHOP primary, secondary and tertiary service areas. (5%)
* Works with physicians to develop processes to increase patient referrals.
* Identifies opportunities for development of new programs, services and affiliate relationships.
* Meets regularly with the CFDT Executive Director and Medical Director, SDU, and Division Chief.

Monitors patient referrals, evaluations, SDU deliveries and NICU volumes. (5%)
* Utilizes Salesforce and Fetal Facts to monitor changes in patient referrals by source and/or geography. Measures the percent change in referrals by tracking physicians/hospitals that have been contacted versus those that have not.
* Develops reporting tool with Business Operations Analyst.
* Develops plan with CFDT Executive Director to intervene where patient referrals decrease.
* Provides CFDT referral report to senior management and/or CHOPPA.
Facilitates referring physician continuing education programs in collaborations with Continuing Medical Education and Physician Referral departments. (10%)
* Represents and/or supports the CFDT at local, regional, national and international conferences.
* Provides Obstetric and Maternal Fetal Medicine Outreach support including individual physician visits, CME events, Grand Rounds and other educational opportunities.
* Acts as primary contact for referrers to ensure their knowledge of CFDT/CHOP systems.
* Provides analysis of market, trends and ROI from conferences/outreach events.
* Meets monthly with CME Manager to coordinate CME efforts.
* Coordinates approximately 3 physician continuing education programs annually in the local area.
* Establishes opportunities for referring physicians to meet CHOP physicians via CME or other designed methods.
* Provides information on a regular basis regarding CHOP sponsored conferences and educational activities for physicians and nurses.
* Identifies innovative outreach methods such as telehealth to raise awareness of CFDT services.
Responsible for development and inventory of collateral materials. (5%)
* Attends monthly review of CFDT Marketing/PR/Web deliverables.
* Ensures timely completion of above materials.
* Facilitates integration of CHOP clinical care guidelines at local community hospitals (as appropriate).

Required Education and Experience
Required Education: BSN. Graduation from an NLNAC and/or CCNE accredited school of nursing or college.
* Minimum of 7 years Obstetric/NICU experience.
* Previous experience in complex clinical settings that required coordination of multiple services.
* Previous experience in teaching and formal educational program development.
Preferred Education, Experience & Cert/Lic
Preferred Education: MSN strongly preferred.
